Pick 4 numbers that tell you whether the plan is working. For each one, fill in where it stands today, where it should be in 90 days and in 1 year, and the one person responsible for moving it.

OutputWhat you did. A count of your own activity: sessions held, people served, lots cleaned up. You control it directly.e.g. 24 sessions held · 600 resident visits
OutcomeWhat changed for people. The difference your work made in someone's life. You influence it but can't force it, and you usually measure it with a short before-and-after survey.e.g. 65% of visitors report less stress

Quick test: Could you hit this number and still leave nobody better off? If yes, it's an output. Include at least one outcome: outputs show funders you're busy, and outcomes show them your work is making a difference.

Tip: Mix result numbers (revenue, profit, repeat customers) with early-warning numbers that predict them (leads, pre-orders, sales calls booked). Early-warning numbers show trouble a month or two before it reaches your bank account.
